help meet the needs of people age 60 and
over who may need help in certain areas

Access: Designated Parking, Full Wheelchair Access, No Stairs in Service Delivery Areas

Services Offered: We have the Community Care Program, Ombudsman, Money
Management, Money Follows the Person, Phones for the Hearing Impaired and SHIP (Senior Health Insurance Program)

Fees: CCP services are funded through IDOA and are no cost to service recipients. Donations are accepted for all other services.

Family Compostition/Income Level/Other: Must be 60 years of age or older and meet eligibility criteria for all
programs,
