Agency Mission: To elevate the quality of life for the older
population by providing a single and familiar
focal point for delivery of services to the
elderly, by offering these services directly or
via linkage to the network of available
assistance; to support older persons in their efforts to live independently in their own homes as an alternative to premature institutionalization and to serve as advocates for the aging population.
Service Summary: Senior services is a program designed to
helpmeet the needs of people age 60 and over that may need assistance in certain areas
to be able to live independently in their
own homes.
Access: Designated Parking, Full Wheelchair Access, No Stairs in Service Delivery Areas
Services Offered: We have the Community Care Program: which is provided through a Home Health Care agency contracted through IDOA
ITAC: These are phones that are free of charge for the hearing impaired.
Ombudsman: Advocates for persons residing in a Long term care setting.
Money Management: This is a program that is part of CCP and is offered in the Effingham, Fayette, Clay, Jasper, and Marion Counties.
SHIP: This is the Senior Health Insurance Program and it provides assistance with Medicare Part D choices.
Fees: CCP services are funded through IDOA and are no cost to service recipients.
Donations are accepted for all other services.
Family Composition/Income Level/Other: Must be 60 years of age or older for all
programs and meet the eligibility criteria.
Geographic Area Served: Marion County thru this office (we also serve
Clay, Effingham, Fayette, Jefferson County
thru our other branch offices.
